Humana is seeking a Housing Navigator to join the Florida Medicaid Market (HHH) team. The Housing Navigator manages and monitors housing or re-housing services for members which involves orienting participants to the program, conducting housing searches, and identifying appropriate community resources and funding. The Housing Navigator also provides supportive services to promote members’ self-sufficiency, establishment within the community, and permanency in housing.The Outbound Contacts Professional 1 represents the company by making outbound contacts to members and/or organizations regarding housing barriers and a variety of issues. The Outbound Contacts Professional 1 work assignments are often straightforward and of moderate complexity.

The Outbound Contacts Professional 1 performs necessary follow up with members and/or organizations. Ensures responses to member and/or organizations inquiries are completed on a timely basis to achieve service and quality standards. Understands own work area professional concepts/standards, regulations, strategies and operating standards. Makes decisions regarding own work approach/priorities and follows direction. Work is managed and often guided by precedent and/or documented procedures/regulations/professional standards with some interpretation.

  • Program participants will be identified through the collaboration of case management by reviewing data on homeless or potentially homeless members and assessing their need for housing intervention services.
  • Develop an individualized housing plan that addresses the member’s barriers to stable housing and identifies interventions.
  • Provide education, guidance, and resources to program participants to support their housing search and retention.
  • Act as a liaison between community housing partners/landlords and members.
  • Conduct housing consultations with program participants.
  • Outreach, engage, recruit, and retain landlords to identify available housing options.
  • Collaborate in the development and maintenance of the housing program, including but not limited to reporting methods, assessments, and development of workflows.
  • Stay abreast of best practices and emerging trends in homeless services, affordable housing, and permanent housing.
  • Maintain caseload of program participants and document assessments, interventions, follow up and outcomes for assigned case load according to the Humana standards.
  • Deliver program overview and process training to staff and program participants on housing development/operating models relevant to homeless services.
  • Serve as a resource for Florida Medicaid Market housing and homelessness initiatives.

Required Qualifications

  • Must reside in the state of Florida.
  • Minimum one (1) year of social service experience working with housing resources and/or with individuals related to their housing needs.
  • Proven interpersonal skills and ability to effectively interact with members and coworkers from a variety of different backgrounds and experiences in a professional and courteous manner.
  • Proven ability to work effectively as a team member and cooperate in achieving market goals.
  • Intermediate level experience with Microsoft Outlook, Excel, and Word.
  • Understanding of technical knowledge
  • Comprehensive knowledge of Microsoft Word, Excel and PowerPoint
  • Strong analytical skills
  • Must be passionate about contributing to an organization focused on continuously improving consumer experiences

Preferred Qualifications

  • Community Health Worker Certification.
  • Knowledge of Medicare and Medicaid programs.
  • Managed Care experience.
  • Bilingual in Spanish.
  • Experience working with persons with disabilities and social determinate of health barriers.
  • Prior experience working within the health service industry and/or service operations
  • Experience with SQL and Access

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
