The Social Determinants of Health (SDOH) Coordinator understands own work area professional concepts/standards, regulations, strategies and operating standards. Makes decisions regarding own work approach/priorities, and follows direction. Work is managed and often guided by precedent and/or documented procedures/regulations/professional standards with some interpretation.

Social Determinants of Health (SDOH) Coordinators (Care Management Support Professional) have regionally based assignments, our coordinators are responsible for finding community-based support to meet enrollee transportation, food insecurity, education, and employment needs, among other SDOH needs. Our SDOH Coordinators will be responsible for supporting Case Managers (CM) and Community Health Workers (CHW) to find appropriate community resources to address SDOH. They will also serve as a resource for Enrollees not in care management to connect with needed SDOH-related services.

SDOH Coordinators conduct and support the following functions:

  • Resource management: Maintaining updated resource list manually and within the Humana Community Resource Directory site for their region
  • Liaise among Humana CMs, providers, and CBOs to coordinate referrals for members to community-based services and programs and to foster integrated efforts among all parties
  • Handle calls from Member Services that require resources and do not have an open CPCM case (can be referred to CM after initial resource assistance)
  • Manage all cases out of SDOH queue by region to outreach, engage, provide resources, and potentially refer for CM 
  • Manage the Unable the Reach (UTC) process:
  • Perform required SDOH and other essential non-clinical assessments
  • Implement initial nonclinical Care Plans and monitor member progress on the member Care Plans
  • Educate members on Value Added Benefits and process those benefit requests for members who meet criteria
  • Preform pre-engagement screening: SDOH Coordinator will provide telephonic outreach to members who are identified through automated means to verify contact information, conduct non-clinical surveys, and offer CM services.
  • Serve as a key knowledge source for community services and information for enrollees and other Humana associates
  • Build and maintain relationships with providers and community resources to support enrollee identification and referrals
  • Implement community assessments to identify community resource gaps
  • Monitor enrollee compliance with their care plan and provide motivational interviewing to support medication and treatment adherence
  • Provide social support to help boost enrollees’ morale and sense of self-worth, serving as a trustworthy, reliable, non-judgmental, consistent, and accepting member of the enrollee’s MDT
  • Support enrollee self-management through the provision of culturally-appropriate health education and health coaching
  • Serve as a key knowledge source for community services and information for enrollees and other Humana associates
  • Conduct research and telephonic outreach to locate difficult-to-contact enrollees to increase assessment completion and participation in clinical programs

Required Qualifications

  • 2+ years experience working with community resources, community health agencies/social service agencies (Area Agency on Aging, DME providers, Meals on Wheels etc.) Case Management experience
  • 1 year of professional work experience
  • Intermediate working knowledge using Microsoft Office Programs specifically Teams, Excel, PowerPoint, Outlook and Word
  • Exceptional communication & interpersonal skills with the ability to build rapport with internal and external customers and stakeholders.
  • Decision making skills regarding own work approach/priorities, and work assignments, standards and resources
  • Ability to multi-task and work in a very fast-paced environment
  • Strong understanding and respect of all cultures and demographic diversity.
  • Strong written communication skills and a strong advocate for members at all levels of care.
  • Location: Must reside in and perform work in Louisiana
  • Hours: Must be able to work a 40-hour work week, Monday through Friday 8:00 AM to 5:00 PM Central Time
  • Training: May 2025

Preferred Qualifications

  • Bachelor’s Degree in Social Work or related field
  • Familiarity with state Medicaid program guidelines
  • Experience engaging with Medicaid enrollees, including those with physical health and behavioral health needs and varied health literacy
  • Fluent in Spanish, French Creole, or Vietnamese

Work At Home Requirements

To ensure Home or Hybrid Home/Office employees’ ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria:

  • At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wireless, wired cable or DSL connection is required.
  • Satellite, cellular and microwave connection can be used only if approved by leadership.
  • Employees who live and work from Home in the state of California, Illinois, Montana, or South Dakota will be provided a bi-weekly payment for their internet expense.
  • Humana will provide Home or Hybrid Home/Office employees with telephone equipment appropriate to meet the business requirements for their position/job.
  • Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.

Section 1121 of the Louisiana Code of Governmental Ethics states that current or former agency heads or elected officials, board or commission members or public employees of the Louisiana Health Department (LDH) who work directly with LDH’s Medicaid Division cannot be considered for this opportunity. Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
