Overview Looking for Social Worker (LCSW) in the Minnesota area (Remote position)! The Care Coach is an integral member of a collaborative care team with a passion for mental and behavioral health, focused on engagement in health and wellness coaching to improve the health and quality of life for members of the Ontrak WholeHealth+ program. Care Coaches help members work on tailored health and wellness goals by uncovering their personal strengths, motivations, and skills. Coaches engage regularly with their members to support them in writing attainable (SMART) goals, provide resources, and support the development of self-efficacy to make lasting behavioral changes. Care Coaches receive training to build on the core concepts of health and wellness coaching to successfully administer clinical and functional assessments of client progress, employ targeted intervention and education, and support provider directed care plans pertaining to clients identified issue(s). Essential Functions Engage and maintain members in active participation in the Ontrak program as well as to coordinate member care. Utilizes motivational interviewing, empathy, and skill-building techniques in their communication with members. Usage of Ontrak program materials and the Ontrak WholeHealth+ electronic documentation platform. Provide exemplary customer service to and member-centric focus for all members. Communicates with providers in complete compliance with confidentiality standards and in support of a collaborative care team relationship Works jointly with the Member Outreach & Engagement and Clinical Operations staff to support member enrollment, referral, and engagement with the Ontrak WholeHealth+ program. Engages in collaborative care team forums to improve quality of care and develop plans for the highest need members Supports enhancement of the program by providing feedback to the development and operations teams and performing ad hoc testing as indicated. Qualifications Required: Extensive experience in a health care consumer-facing environment; experience performing assessments and implementing care plans; using motivational interviewing to help change behavior Extensive experience in a clinical setting working with Serious Mental Illness population Experience in Medicaid Program Work Location: 100% remote position but must reside in Minnesota License Required: Clinical License Social Worker (LCSW) Empathy: You embody humility and integrity by humanizing people in sensitive settings and demonstrating support.
