We’re looking for a hands-on supervisor for our Care Manager team as they build relationships with families with adult neurodivergent children — helping them access specialized resources. We specialize in helping families navigate a specific California state funded program called Self-Determination, which offers more creative and custom choice as to how to leverage state funds.  

This is a life-changing program and you will essentially be the captain of our virtual Care Manager team, guiding them to effectively manage cases — ensuring our client families both love our services and achieve their goals. You will be the driving force behind the Care Managers, setting goals and monitoring their seamless execution of custom client plans and step-by-step processes to guide families as they enter and maintain participation in this program.  Youll be responsible for assigning cases to your team, reviewing their work, and providing feedback — all while keeping a keen eye on key metrics to prove the value we deliver. Youll serve as the managers’ go-to person for daily challenges they might encounter as they advocate for their clients, taking decisive action on cases or team issues. In this remote role, you will be able to leverage your creativity and leadership skills in social work, case management, and service coordination.

Last year, we supported hundreds of individuals in accessing life-changing services, and we’re on track to quadruple our impact next year.

Responsibilities include:

  • Team Leadership: Building and managing a high-performing team of remote care managers, providing coaching, mentoring, and setting clear goals to achieve outstanding client outcomes. Assist in the hiring process of new Care Managers.

  • Strategic Decision-Making: Optimize performance and outcomes by analyzing and driving team metrics like retention rate, Net Promoter Score (NPS), and customer lifetime value to measure performance and identify areas for improvement.

  • Client Advocacy: Ensure a strong partnership and effective collaboration by representing NeuroNav in client and partner meetings.

  • Company Policy Improvement & Implementation: Execute and uphold policies and procedures to ensure the teams success in guiding individuals through the Self-Determination Program. 

  • Optimization: Identify areas of improvement and work together to implement positive change. 

  • Collaboration: Work closely with sales, marketing, and leadership team to streamline the customer journey and ensure seamless handoffs throughout the client lifecycle.

  • Talent Development & Training: Identify and nurture the growth of future leaders within the team, building skill sets and fostering a culture of mentorship. Host training for new Care Managers.

What You’ll Bring:

  • Team Management Experience: Demonstrated success in leadership and professional development of teams, including motivating team members to achieve individual and collective goals while fostering a positive and collaborative work environment (2+ years of management experience required)

  • Developmental Disabilities Experience: 2 years of IDD support experience is required and 3+ years is preferred.

  • Background in Case Management is required

  • Bachelor’s Degree or equivalent work experience

  • Client-facing experience managing multiple relationships at one time

  • Excellent written and verbal communication skills

  • Must have computer, reliable high-speed internet connection

  • Fluent in Spanish and bilingual (strongly preferred)

  • Experience in a performance-based culture with metrics attainment goals

  • Experience with Microsoft Office & Google Suite

Salary: $70-90k per year (depending on experience)
